Jack's story is one of resilience, determination, and the power of specialised rehabilitation combined with amazing family support.

After experiencing a life-changing event in February 2021, Jack and his family embarked on a rehabilitation journey to maximise his physical independence. This case study explores his condition, the interventions provided by PhysioFunction, the unwavering family support and encouragement and his remarkable progress so far.

Jack’s History and Initial Challenges

On February 17th, 2021, Jack experienced a catastrophic health event. He suddenly collapsed at home and was rushed to Queens Medical Centre in Nottingham, where scans revealed a significant brain bleed caused by a ruptured arteriovenous malformation (AVM). During emergency surgery to remove part of his skull and relieve pressure and stop the bleeding, Jack suffered a stroke.

Following this event, further investigations revealed the presence of three additional AVMs. One was successfully removed surgically, while the remaining two are being carefully monitored. Jack’s medical crisis led to an extensive hospital stay of seven months, followed by 14 weeks at The Children’s Trust, a specialised brain injury rehabilitation centre in Surrey.

During his time at The Children’s Trust, Jack made impressive strides in regaining strength and mobility. However, once he returned to the community, there was limited availability of specialised rehabilitation services. Jack’s family wanted to continue this specialist neurological physiotherapy and were signposted by another PhysioFunction client Jack’s family had met whilst in the Children’s Hospital in Nottingham.

Jack began his journey with PhysioFunction in May 2022.

Initial Presentation at PhysioFunction

When Jack began therapy at PhysioFunction, he presented with significant physical challenges:

  • Left Upper Limb: Jack’s left arm showed no functional activity or voluntary movement. It was hypersensitive, causing pain during handling, and Jack actively resisted attempts to move it.
  • Left Lower Limb: Jack had tight muscles in his left leg, his knee was stiff and hard to straighten, and he had restricted range of motion, and swelling in the foot.
  • Posture and Mobility: Jack’s sitting balance was poor, and he was unable to stand or reach out of a seated position. Transfers at home and school required a hoist and attempts to use a slide board involved the assistance of two people. He was unable to stand.
  • Overall Functional Ability: Jack relied entirely on support for all mobility and activities of daily living.

Interventions at PhysioFunction

PhysioFunction implemented a tailored, physiotherapy program to address Jack’s unique needs and goals. His sessions focused on:

1. Core Strengthening and Balance Training

  • The Clinical team focused on improving Jack’s core strength to enhance his postural stability. Over time, his sitting balance improved significantly. He is now able to sit unsupported, maintain head control, and reach for objects outside his base of support.
  • These improvements laid the foundation for more advanced activities.

2. Left Arm Desensitisation and Passive Movement

  • Through gradual and consistent handling during sessions, Jack has become more tolerant of touch on his left arm.
  • Passive movement exercises were introduced, encouraging greater relaxation and engagement with his left upper limb.

3. Lower Limb Strengthening and Stretching

  • Jack tolerates stretches to reduce muscle tightness and improve range of motion in his left leg.
  • Strengthening exercises for his right leg complement efforts to improve overall mobility.

4. Progressive Standing and Walking Training

  • Early sessions used a tilt table to assist Jack into a standing position. As his strength and understanding of how to push towards standing improved, he progressed to using an Easy Glider and standing frame. Jack is now trying to stand using a Molift or the parallel bars with two people supporting.

  • Jack has also independently propelled the Meywalker through the clinic.

Jack’s Progress and Achievements

Jack’s dedication and hard work, combined with the PhysioFunction team and his family’s support and encouragement, have resulted in amazing progress:

  1. Enhanced Mobility
    Jack has achieved - walking with support. The LiteGait system allows him to take purposeful steps with improved extension and head control. Walking has become a highlight of his therapy sessions, reinforcing his motivation and enthusiasm.
  2. Improved Sitting Balance and Posture
    Jack can now sit independently, lift his head, and interact with his environment in a more controlled and confident manner.
  3. Increased Engagement with Left Arm
    His tolerance for handling and passive movement in his left arm has improved significantly, opening the door for further functional gains in the future.

Next Goals

Jack’s next major milestone is to achieve standing transfers. This functional improvement would increase his independence and with the team at PhysioFunction alongside his family being focused on continuing to build strength, balance, and coordination we have no doubt Jack will reach this goal.
